Why counts, and not streaks
Every habit app in existence would show you a streak here. This one counts instead, and the difference is not cosmetic.
A streak turns one hard day into evidence of failure. You miss a Tuesday because you could not get out of bed, the number resets to zero, and the app you installed to help now agrees with the part of you that says you cannot keep anything going. People abandon trackers at exactly the moment they most needed one, and this is why.
"Done nine times in the last thirty days" cannot be reset by a bad week. It is also the more honest number: nine times is nine times, whether or not they were consecutive.
The same applies to the second list. Write down the things that were harder than they looked — ringing the surgery, answering a message you had been avoiding for a fortnight, getting to the end of a day. In November you will not remember that in October you could not have done it.
